I acknowledge that dance classes involve physical exertion and carry the risk of injury. I certify that I am physically fit to participate and have not been advised otherwise by a medical professional.*

I hereby release Buckle Bunny Line Dancing/Haylie Petho and any other Buckle Bunny Team Members from any claims, demands and causes of actions arising from participation in the dance class.

I hereby release Buckle Bunny Line Dancing/Haylie Petho or any other Buckle Bunny Team Members from any claims, demands and causes of actions arising from my Child participation in the dance class. I take full responsibility for the safety and wellbeing of my own child or other minors that I bring to class.*

Buckle Bunny Line Dancing will often film classes for promotional purposes. I give permission for videos and photos of myself to be used for promotional purposes. *
